After a claimant is injured, their employer is supposed to file a first report of injury, which is then normally forwarded on to the insurance carrier and to the Division of Workers’ Compensation. This form acts as the employer’s version of the injury and is used by the insurance companies to initially set up the file and begin working on the case. To review an Employer’s First Report, log on to the Division of Workers’ Compensation website (http://www.coworkforce.com/dwc/) and go to “Forms”. (Section 8-43-101, C.R.S.).
